Your First Visit

Here's what to expect

  1. When you arrive, we'll text you a link.
  2. Click the link in the text message to begin the check in process.
  3. Confirm your identity with last name and date of birth and indicate whether you are the patient, or representing the patient.
  4. Select the appointment for which you are checking in.

Review Demographic Information

Review and update your information, scrolling if necessary, and clicking Next to progress through the screens.

Manage Insurance and Identification Information

Manage Insurance

On the Insurance screen, select one of the following options, then click Next.

  • Add Insurance
  • Provide Insurance at Visit
  • Do Not Have Insurance

Manage Identification

If you do not have any identification information on file you will be prompted to:

  • Add a photo of your ID card, OR
  • Indicate that you will provide it at time of visit, OR
  • Indicate that you do not have identification

Once you make a choice, the Next button is enabled.

Complete Consents and Intake Forms

Complete the consent and intake forms.

Payment Options

You have the option of paying online or paying at the front desk upon arrival for your appointment.

Any Copay amount must be paid in full, but partial payments on outstanding balance are allowed.

Complete Check In

Once you have progressed through the review of demographic and insurance information, completed consents/intake forms, and completed payment, check in is complete!
