Your First Visit

Information We'll Need

At the time of your first visit, we will need to collect several pieces of information. Please be patient with us, as your information is very important to us. It will allow us to serve you better with patient centered care. It is our goal to make this process as easy as possible.

General Information:

  • Patient’s Social Security Number
  • Insurance policy holder's Social Security Number
  • Insurance policy holder’s date of birth
  • Insurance card or copy of both the front and the back of the card
  • Insurance referral if required by your insurance company

Medical Information:

  • List of medications the patient is presently taking
  • Previous surgeries and health issues
  • Family History
  • Any X-rays, CT scans, or MRI scans you have had that pertain to a possible injury.

What you can expect at your first visit

At your first visit, you will need additional time with our staff to collect all of your pertinent information. You will be seen and evaluated by our physicians with physical examination. Our physicians may request further imaging (X-rays or MRI) if warranted. A formal treatment plan will be detailed, giving the patient all reasonable options with benefits and risks to each. You will be an active participant in your treatment plan, ensuring the plan is tailored to your preferences.

Work Related Injuries

If you are being seen for a work-related injury, you need to obtain a letter from your employer stating that this is a work-related injury; the letter must include a billing address.
